Jasmine is an alive and vibrant community that offers loving, dignified, and excellent care in a home-like environment that enables people to celebrate abundant lives. We take care of individuals 60 years or older. Residents with a dementia diagnosis may also be accepted and retained.

Here are the services we provide to keep your loved ones happy and comfortable in our homes:
- Care and supervision
- Assistance with self-administered medications
- Observation for changes in physical, mental, emotional, and social functioning
- Notification to resident’s family, physician, and another appropriate person/agency of resident’s needs
- Lodging: Private (single) room and semi-private (double) room
- Food services, to include 3 nutritious meals daily (daily home-cooked meals using only the freshest ingredients) and snacks, special diets if prescribed by a doctor
- Assistance to gain access to supportive services, such as voting, senior assistance agencies, etc.
- Transportation – We will provide transportation within a 10-mile radius at least 4 times per month
- A planned activity program including arrangement for utilization of available community resources as identified in the attached House Rules; and
- Assistance with personal activities of daily living as follows:
- Dressing
- Eating
- Toileting
- Bathing
- Grooming
- Mobility tasks
- Other personal care needs
Additional basic services included in the Basic Services fee that the resident needs or wants and will be provided by the facility include those below:
- Hygiene items of general use, such as soap and toilet paper
- Laundering of personal clothing
- Clean bed and bath linens weekly, or as often as needed
- Cleaning of resident’s room
- Comfortable and suitable bed and/or bedroom furniture
- Assistance in meeting necessary medical and dental needs as follows:
- Assistance with taking prescribed and over-the-counter medications following physician’s instructions unless prohibited by law or regulations.
- Bedside care and tray service for minor temporary illnesses or recovery from surgery.
